Can I go to any doctor I choose? |

Yes. With a Preferred Provider Organization (PPO), you select the physician or other health care provider you
want to use. Remember, though, that choosing a provider from our network will
lower your out-of-pocket expenses and save you money. On our
provider search page, you have access to our entire Provider network
with a wide selection of the best family physicians, specialists, and
hospitals. |

Why do I have more than one PPO network listed on my card? I thought Significa
Insurance Group (Significa) was my PPO. |
| A. |
Significa with the EHP network option is
your PPO. Significa has direct contracts with physicians and hospitals across portions of Pennsylvania,
while in other areas we have indirect arrangements with providers by contracting with another PPO. If you see a logo
other than EHP, it means Significa is "partnering" with another PPO in that area. Since the provider has a direct contract
with that partner PPO, it is necessary to have them listed on your ID card along with the EHP logo.

Do I have to ask permission to see a specialist? |

Not at all. There's no "gatekeeper" physician to go through.
Select care whenever you need it from any provider you want. Remember, though,
that choosing a provider from within our network will lower your out-of-pocket
expenses and save you money. |

What if I need health care outside my immediate area? |
| A. |
Our expanding service area includes most counties in Pennsylvania. For
assistance in identifying providers associated with our network, go to our
provider search page. But even if you find yourself needing emergency
health care outside our network, you're always covered. If you are admitted to
a hospital, remember to follow the directions on the back of your ID card, or go to our
Payer Partners page for contact information. |
